Vacation pay will be included in your paycheck on the last pay date of the month on which your anniversary of employment with Salt Lake Brewing Co., L.C. occurs. The employee must be employed, in good standing, on the anniversary date to be eligible for this benefit.
On the first and second anniversaries, you will be eligible to receive vacation pay equal to the average number of hours worked per week, up to a maximum of 40 hours.
On the third and fourth anniversaries of employment, you will be eligible to receive vacation pay equal to the average number of hours worked per week multiplied by 2, up to a maximum of 80 hours.
On each anniversary of employment beginning with the fifth year, you will be eligible to receive vacation pay equal to the average number of hours worked per week multiplied by 3, up to a maximum of 120 hours.
If the employee voluntarily resigns or is terminated prior to his/her anniversary date, no vacation pay will be paid.
Group insurance benefits for health insurance (including prescription drug program and employee assistance plan) and dental insurance are available to tipped and hourly employees after 12 continuous months of full-time employment and average of 32 hours per week. Currently, Salt Lake Brewing Co., L.C. pays a portion of the premium costs, and the employees pay a portion.
Additional details about these benefits will be distributed to you upon eligibility or during the open enrollment period. It is recommended that you sign up for the insurance benefit when you become eligible, otherwise if you later decide you want coverage there may be additional waiting periods.
